Admissions & Referrals: Residential- HOW WE WORK WITH YOU

Pomegranate takes referrals from physicians, shelter care, department of children service, schools, probation office, social work and mental health professionals.

Each patient and family is welcomed through the intake and assessment process.

Pomegranate representatives inquire about the child's previous treatment, clinical history and family information.

To assist in the referral process, download, print and complete our residential referral/admission packet and fax or scan and e-mail it to us along with the requested documentation. You'll find instructions for emergency shelter care admissions, or residential admissions to accompany the packet for both.

Admissions & Referrals- Acute:

Adolescents are admitted by physician order. Their care and treatment are directed and supervised by attending psychiatrists and their multi-disciplinary treatment team.

Admission is limited to adolescents (ages 12-17) whose principle diagnosis and focus of treatment upon admission is a mental health disorder according to the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders (DSM IV-TR, or subsequent editions).

The admissions coordinator/nurse obtains demographic information, presenting symptoms, as well as other relevant clinical information. If the patient meets minimum criteria, the admissions coordinator contacts the physician on duty to present the patient. Physician evaluates the referral and renders a decision within 30 minutes of initial contact with the agency. Call our acute nursing intake personnel at 1-800-476-0868.
